The decision was based in whole or in part on information received from the consumer reporting agency, ProviderWatch, LLC. ProviderWatch, LLC did not make the decision and is unable to explain why the decision was made. You have the right to request a free copy of your report from ProviderWatch, LLC within 60 days from the date of this notice, May 18, 2024.

You have the right to dispute the accuracy or completeness of any information provided by the consumer reporting agency.

If you take adverse action (e.g. deny the application, require specific payment terms, etc.) based on information in a consumer report, you are legally obligated to tell the consumer to comply with the Fair Credit Reporting Act. Your notice may be oral, written or electronic; it must contain certain information, specifically the name of the consumer reporting agency, address and toll-free number for the agency and the applicant’s right to request a copy and dispute the accuracy or completeness of the information contained in the report.
